Go Yoon-jung, who portrayed the first-year resident Oh Yi-young, expressed her bittersweet feelings on May 18, stating, "Every moment in this project was precious and meaningful, making it hard to say goodbye. I sincerely thank the viewers who loved this work." She added, "Playing Yi-young allowed me to learn and grow, and I believe this project has become a significant turning point in my career. Shin Chae-kyung, who played the character Pyo Nam-kyung, shared her gratitude, saying, "It was an honor to finish my first drama meaningfully. Every moment spent as Nam-kyung was incredibly happy and valuable. I sincerely thank everyone who loved the drama and Nam-kyung, and I will repay you with even better performances in the future."

Kang Yoo-seok, who took on the role of the former idol Um Jae-il, reflected on his experience, saying, "Portraying Jae-il, who is caring and persistent, brought me a lot of positive energy. I met precious people through this project and grew both as an actor and as a person." I hope my character was able to share bright and positive energy with the viewers."

Han Ye-ji, who made her drama debut as the super AI Kim Sa-bi, expressed her joy in being part of the cast, saying, "It was an honor to belong here as Sa-bi, and I was so happy to be with Yi-young, Nam-kyung, Jae-il, and Do-won." She also acknowledged the support from the staff and production team, stating, "I was worried about not being able to contribute well, but thanks to the help from the staff and crew, I was able to finish safely. I am truly grateful."

Finally, Jung Joon-won, who played Gu Do-won, shared his excitement about the experience, saying, "These six weeks passed by so quickly, and it was the most thrilling and happiest time of my life. I wish all the viewers who have supported us good health and happiness."

'Once Upon a Time in Medical School' tells the relatable story of residents as they navigate the challenges of their first year.
